- We provide medical consultations and services to you based on information you provide to the questionnaires you complete, and then Face to Face with a Doctor or Clinician
- You warrant and undertake to complete the consultation questionnaire relating to your condition contained on this website truthfully and honestly, disclosing all relevant information truthfully and to the best of your knowledge.
- If we determine based on our medical assessment and face to face consultations that you are suitable for treatment we will issue a prescription
- The general information provided on our website is only a guideline and should not be relied completely. Where you agree, we keep your GP informed.

We warrant and undertake that your consultation questionnaire will only be viewed by a healthcare professional Clinician or a Doctor and his staff for the purpose of treating your medical condition
- You are not allowed to register more than once;
- Please do not register or complete questionnaires on behalf of anyone other than yourself, unless you have permission to do so from the individual and we receive express verbal consent from the individual;
- We cannot and are not liable for any damages which results from: (i) a failure by you to provide (and continuously maintain) complete, truthful and accurate information in your Patient Record and in all questionnaires you complete or any other information you provide to Us either verbally or in writing; (ii) a failure of you to follow advice given on the website or from your failure to pass on information given on the website to your regular healthcare provider. You accept that the advice given on the website does not replace your regular healthcare provider. You must tell your regular healthcare provider about treatment we prescribe and medication supplied by the Medic Plus Clinic

Who or what has caused you concern, Where or when the event took plac,What action, if any, has already been take,What action you hope your complaint will achieve.
The Clinic Lead will acknowledge receipt of your letter within 2 working days.
Complaints relating to medical management by an individual doctor or nurse.
In cases where the complaint refers to the management of a medical condition by a particular doctor or nurse, the complaint will be passed on to that doctor or nurse. He/she will then be responsible for responding to the patient’s complaint. They should acknowledge receipt of the complaint within 7 days. Most doctors and nurses will then consult their professional insurance company (usually the Medical Defence Union or the Medical Protection Society) before producing a full response and these organisations can be very slow to give advice, so this may take some time. In some cases these organisations may ask to contact the patient themselves. MPC will endeavour to expedite this process throughout but is necessarily limited in its ability to influence the time taken by indemnity organisations, and we will do our best to keep the patient informed of progress.
Where the complaint also refers to the service offered by MPC this will be addressed separately as above.
